Attached to this article is a flyer we made to help our folks quickly and easily access the online forms we use to collect information for claims and appeals.
We have a few members who use our online forms quite a bit, and many members who seldom use them at all. We've also put together two new forms to help us get better information for appeals and arbitration, but these forms have yet to be used by our members.
With this flyer, our members can take a quick snapshot of the appropriate QR code with the camera on their smartphone. This will prompt them to open a link that takes them directly to the claim form they want!
The rules and contracting claims forms still require a member to register to our website for access. This is important to ensure we can match claimants properly and ensure we don't get bogus submissions.
The two new forms DO NOT require a website registration, anyone can access and submit. These forms are more for providing additional evidence or support for existing claims, or to help us develop a database of skills our members possess (to challenge the Carrier's arguments that our members aren't trained, skilled, or experienced to do the work they prefer to contract out). We can't just rely on past statements or simply telling they're wrong! We have to back up our arguments with statements from our members demonstrating that we know our craft.
So, print this flyer out and keep it handy. You can pass around additional print-outs as needed and try out the QR codes yourself. I will make laminated copies that will go out with the next quarterly report, so you have a durable copy in case you need it.
One caution. Don't make photocopies of this document. If you need more, print from the original file. Copies may make the QR codes fuzzy and so cause them to fail.
